According to Adam Schefter of ESPN, Incognito was summoned to the league office prior to the 2012 season to discuss off-field incidents that apparently never made it to the public eye. (One of those incidents surely related to the golf-course incident that came to light this week.)
Schefter also reports that Incognito was involved in a June 2013 fight at a night club. Incognito reportedly punched bouncer Carlos Joseph (the brother of former NFL defensive tackle William Joseph), Incognito was punched as well, and security cameras showed Incognito spitting out blood.
And lets not forget that Incognito allegedly punched and knocked out a security guard at an incident before the start of 2013 training camp.
These past incidents quite likely will be used by the Dolphins to support the suspension of Incognito as the straw that broke the camels back.
So one of them surely was the golf incident even though another source said that it had been reported to the league. I guess that it is so then.
EDIT: So it did not make it to the public eye does not mean it was not reported to the NFL office and does makes sense they would talk to him about it. It does not mean that the Phins hid the incident from the NFL even if it was kept from the public.
I think one incident is being counted as two here.
The 2013 security guard incident, is that the one where Incognito had injuries but that the guy he supposedly knocked out had none according to the police report and where he was the one who called the police in and in which others backed up his story that he did not throw a punch?

Dolphins left guard Richie Incognito received a trespassing warning on June 10th after an alleged fight with a security guard at Club Liv in Miami Beach, according to a police report first obtained by The Palm Beach Post.
Incognito and some friends got into a skirmish with Carlos Joseph, a security guard who has the same name and birth date as the former University of Miami and NFL right tackle. Joseph played for UM from 2001-2004 and was on San Diego and Jacksonville’s rosters from 2004-2006.
The report said that Incognito and friends of his were trying to get close to a performer on stage when Joseph asked them to step back. Somebody from Incognito’s group allegedly said to Joseph “f— you and what are you doing to do about it?” and pushed Joseph.
Incognito told police that he was trying to break up the fight when he got hit in the face by Joseph.
Incognito had minor injuries to his face and refused medical treatment. Incognito received a trespassing warning by Miami Beach police and “advised not to return,” according to police. ...

We passed along a report today that Dolphins guard Richie Incognito punched and knocked out a hotel security guard in an incident before the start of training camp. We now have some more information about the matter.
PFT has obtained a copy of the police report connected to the incident, and Incognito’s side of the story was that he was merely trying to break up a fight.
The incident took place when Incognito and his friends were attempting to get close to the stage during a performance at a club in the Fontainebleau hotel in Miami. A security guard told police that he asked Incognito’s group to step back, and someone in the group replied, “f–k you and what are you going to do about it?”
At that point, an altercation started between the security guard and Incognito’s friend, and Incognito told police that he was just trying to step in and stop the fight when he was hit in the face by the guard. The police report says Incognito had minor injuries to his face but declined medical treatment.
Incognito was not arrested but was escorted from the club and given a trespass warning, and he was told not to return to the hotel without permission.
